Home›Baltimore City

Baltimore City's assisted living looks nothing like the brochure image of a single big building. Most licensed programs here are converted rowhouses and detached homes in the northwest and northeast of the city, each licensed for a handful of residents and almost all approved at Maryland's highest level of care. That gives families a lot of choice at the small end, but it also means quality varies house by house, and the license, the delegating nurse and the written disclosure statement matter more than the curb appeal. The handful of large communities sit mostly near the city line.

Why does Baltimore City have so many small assisted living homes?

Maryland licenses assisted living by bed count and level of care rather than by building type, so a rowhouse licensed for four or eight residents is regulated under the same COMAR 10.07.14 chapter as a 120-bed community.

Small homes can offer a household feel, Medicaid waiver participation and lower monthly rates, but they rely heavily on one manager and one delegating nurse. The small home vs. large community comparison walks through what to check.
